What Causes a Hiatal Hernia? Who Is at Risk for Hiatal Hernia? How Is a Hiatal Hernia Diagnosed

  Hiatus hernia

Hiatus hernia is a very common disorder occurring in about 60 percent of people by the age of 60. It is the bulging of the upper part of the stomach up to the chest. The muscles, which separate the lungs from the abdomen, are called as the diaphragm. Hiatus is a small hole that is developed in the diaphragm. Hiatus hernia occurs when this hole in the diaphragm expands or weakens. As a result of this, a portion of the stomach moves up in the chest cavity

Hernias can only be repaired by a surgical procedure. There are a variety of surgical procedures available, depending on the type of hernia involved. Careful diagnosis is essential.

  Types
 

There are three types of hiatus hernia:

  • Sliding: the portion of the stomach slides in and out of the chest.
  • Fixed: the upper part of the stomach gets caught up in the chest.
  • Complicated: is uncommon but may prove to be serious and at times the patient may also be required to be operated on.
   
  Symptoms
 

This mainly occurs due to obesity, constant coughing, heavy lifting, constipation or poor seating posture. It may be even hereditary. The most common symptoms of hiatus hernia are heartburn, acid reflux and difficulty in swallowing food. However, sometimes there may not be any symptoms at all. The diagnosis of this hernia can be done by a gastrointestinal barium x-ray. Gastroscopy is also a complimentary test.

   
  Effects
  A lot of complications can develop if the symptoms aren’t taken care of. Chronic heartburn, wheezing, ulceration and pneumonia are some of the complications. Chronic anemia may also be developed. In the elderly, it can cause intense chest pain and a difficulty in breathing.
   
  Symptoms
 

The symptoms of hiatal hernia are usually the same as those for gastroesophageal reflux disease. These include:

  • Heartburn
  • Undigested food that comes back up
  • Belching up a sour substance
  • Nausea, especially in the morning
  • The sensation of something in the throat
  • Severe chest pain after eating.

  Types of Procedures
 
 

The Conventional Method. In this case, an incision is made over the site of the hernia. The protruding tissue is returned to the abdominal cavity, and the sac that has formed is removed. The surgeon repairs the hole or weakness in the abdominal wall by sewing
strong surrounding muscle over the defect. This is the most common method of hernia repair.

Tension-free Mesh Technique. For this technique, an incision is made at the site of the hernia and a piece of mesh is inserted to cover the area of the abdominal wall defect without sewing together the surrounding muscles. Recovery is swift, and the likelihood of the hernia recurring is small. The mesh is safe and generally well-accepted by the body’s natural tissues.

The Laparoscopic Method. A laparoscope is a long metal tube with a fiberoptic light source and a telescopic eyepiece, which is connected to a TV monitor. The scope is inserted into the abdominal cavity through a small incision and is used to view the hernia in the abdominal wall while the surgeon repairs the hernia through additional tubes that are inserted into the abdomen through separate incisions.
